Maintaining the confidentiality of your personal information is very important to you and to us at Azzad. We do not sell your personal information to anyone and we do not disclose it to anyone except as permitted or required by law or as described in this statement.
INFORMATION WE COLLECT.The Funds collect the following nonpublic personal information about you, including:
- Information the Funds receive from you on or in applications or other forms, correspondence, or conversations (such as your name, address, phone number, social security number, date of birth and beneficiary information);
- Information used to verify your identity;
- Information about your transactions with the Funds, their affiliates, or others (such as your account number and balance, payment history, parties to transactions, cost basis information, and other financial information).
- Information about any bank account you use for transfers between your bank account and your Azzad accounts; and
- Information we receive about your or your accounts as a result of your inquiries by mail, email or telephone.
INFORMATION WE SHARE AND YOUR RIGHT TO “OPT-OUT”. Information on your Azzad Funds accounts registered under the same social security number or taxpayer ID number and identical mailing address is generally combined in one quarterly or annual statement. We, therefore, share your personal information with those individuals with whom you share a consolidated account statement.
If you prefer that we do not disclose your personal information by means of a consolidated account statement and instead receive an individual account statement for each account, you may opt out, that is, you may direct us not to consolidate your account information. If you wish to opt out, please contact Mutual Shareholder Services toll-free at 888.350.3369.
If you are a joint account owner, we will accept directions to opt out from any one of you and will apply it to the entire account.
INFORMATION WE DISCLOSE.The Funds do not disclose any nonpublic personal information about current or former shareholders to affiliated or unaffiliated third parties, except as required or permitted by law. The Funds are permitted by law to disclose all of the information it collects, as described above, to its service providers (such as the Funds’ custodian and transfer agent) to process your transactions and otherwise provide services to you. Such companies are not permitted to use or disclose your information for any other purpose. All of the information we collect may be share among the Azzad companies, all of which are engaged in the financial services business. Finally, we may disclose information about you at your request (for example, by sending duplicate account statements to someone you designate), or otherwise as permitted or required by law.
INFORMATION SECURITY.The Funds restrict access to your nonpublic personal information to those persons who require such information to provide products or services to you. Azzad employees are trained to follow our procedures to protect your privacy and are instructed to access information about you only when they have a business reason to obtain it. The Funds maintain physical, electronic, and procedural safeguards that comply with federal standards to guard your nonpublic personal information.
WHAT YOU CAN DO? To protect your nonpublic personal information, we recommend that you do not provide your account information or Azzad Funds’ user name or password to anyone.
